Acceptable Identification Documents:
- Drivers License or ID card (issued by California or other state).
- Identification card issued by any branch of Armed Services of USA.
- A passport issued by the USA or issued by a foreign government which has been stamped by the United States Immigration.
- A drivers license issued by a Canadian or Mexican public agency authorized to issue drivers licenses.
- An inmate identification card issued by the Dept. of Corrections, if the inmate is in custody. (An ID bracelet is NOT adequate)
- An employee identification card issued by an agency or office of the State of California, or by an agency or office of a city or county in California.
All of the above ID is acceptable as long as the document is current or has been issued within 5 years.
The ID must contain a photograph, a description of the person, signature of the person, and must bear a serial or other number.
If you cannot produce one of the above, you will have to bring 2 credible witnesses. Those 2 must present satisfactory ID and cannot have a financial interest in the document being signed. Credible witnesses will
be asked to take a verbal oath.
If the signer is unable to sign his name, they may sign with an (x). This must be witnessed by 2 persons.
The approved identification CAN contain more information but never less than the requested name to be signed. Your ID must reflect the same name that you will sign on the document.
Example: Sign as John T. Smith, III. ID has John T. Smith
(not acceptable because ID does not have III on it)
